Abstract

The GENIUS system occupies a unique functional position in acute dialysis. The mobile single-pass batch platform enables intermittent hemodialysis (IHD), sustained low-efficiency dialysis (SLED) and continuous veno-venous hemodialysis (CVVHD) for up to 24 h. The dialysate is centrally generated by reverse osmosis and filled into the system; no dialysis water supply, mobile reverse-osmosis unit or wastewater connection is required at the bedside. By discontinuation of the entire system, modality flexibility and a resource-sparing option with minimal bedside infrastructure are lost. Alternatives are traditional bag-based continuous veno-venous hemofiltration (CVVHDx) or conventional IHD/SLED, the latter requiring bedside water supply and wastewater disposal infrastructure. For 1500 treatment days, the modelling of the resource needs for CVVHDF resulted in 9.0-fold more packaging volume and generated 22.1-fold greater transport weight compared to the GENIUS system. As based on the current evidence continuous and prolonged modalities show comparable patient-relevant outcomes with respect to fixed endpoints, the gaps in treatment should be closed using a mobile successor platform with integrated regional citrate anticoagulation.
