Abstract

In recent years, both the capabilities and the availability of endovascular emergency treatments provided by interventional radiology have expanded significantly. This development has led to a broad range of emergency indications in which endovascular therapy is now considered one of the preferred treatment options. At the same time, a 24-hour interventional oncall service has become an indispensable part of everyday clinical practice. While this represents a highly positive development for the field, it increasingly pushes routine operations to their personnel and organizational limits. This raises the central and at the same time sensitive question of which emergency interventions truly require immediate attention during the night hours between 10 p.m. and 7 a.m., and which procedures can be postponed safely until the following day. The following article presents six case examples and their respective clinical courses, analyzed with particular focus on emergency management. In addition to the course of events and technical implementation, the discussion will especially address the assessment of the temporal urgency of treatment. Key Points The urgency of emergency endovascular interventions depends on the location, morphology, hemodynamics, and coagulation status. Hemodynamically significant abdominal arterial bleeding requires immediate interdisciplinary assessment and, as a rule, prompt and often endovascular treatment.
