Non-functioning pituitary or hormonally inactive tumors are usually slowly growing tumors, which do not cause specific syndromes due to the overproduction of hormones. Patients do not notice the lesions, until the tumor's mass effect causes loss of vision, diplopia, headache or endocrinological disturbances. Since there is no reliable anti-proliferative medical treatment available, surgical interventions are required in symptomatic tumors. For surgery, the transsphenoidal approach is most frequently used. It is safe and efficient, as long as there is a sufficiently wide connection between the intra- and extra- sellar portions of the tumors. A radical resection is attempted, but not always possible. Modern advances, such as the use of the endoscope, intraoperative imaging and neuronavigation, decrease the surgical risks and increase the extent of resection. Craniopharyngiomas and parasellar meningiomas are frequently attacked via craniotomies. Various management strategies have been discussed for the non-adenomatous sellar and perisellar lesions. Repeat radiological, endocrinological, and ophthalmological evaluations are necessary for patients in the follow-up with non-functioning pituitary tumors in the preoperative and postoperative period.
