Abstract

In Western World countries pulmonary embolism up to 42 days after delivery is the leading direct cause of death in women. We report of a 31-years-old woman with severe peripartal pulmonary embolism in her 31st (30 + 4) gestational week. Hence, a primary caesarean section followed by conventional anticoagulation was performed. As this proven to be insufficient as observed by deteriorating haemodynamic parameters a systemic thrombolysis with alteplase was successfully carried out.
