Abstract

Background: According to current guidelines and recommendations incurable cancer patients should receive palliative care as needed in a structured manner, whether as part of general palliative care (APV) or by referral to palliative care specialists (SPV). Question: Various concepts for integrating palliative care are presented and reviewed for feasibility. Material and methods: A literature search and analysis of guidelines and recommendations for action were carried out and in addition a discussion of underlying concepts against the background of a new palliative care metric is presented (symptom and needs screening with minimal documentation system/integrated palliative outcome scale, MIDOS/IPOS). Results: The concept of a stage-related early integration of palliative care (“early integration”) is increasingly being replaced by a screening-based, needs-oriented timely integration of palliative care (APV and SPV, “timely integration”). The feasibility of symptom and distress screening with palliative care self-reporting questionnaires (MIDOS or IPOS) is currently being piloted in German Cancer Society (DKG) certified organ centers and cancer centers. Conclusion: The structured recording of patient-reported symptoms and distress as well as appropriate assessment and intervention concepts contribute to tailored palliative care.
