Abstract

Emergency medical treatment in Germany is increasingly being centralized in emergency care units and this process is accelerated by the settlement of emergency practices of the Association of Statutory Health Insurance Physicians (KV) on or in hospitals; therefore, in these emergency centers the numbers of patients with low-priority injuries as well as life-threatening emergencies are continuously increasing. The centralization has not yet led to the situation where both sectors of emergency medicine can reliably interlock and coordinate their treatment processes. In this situation patient safety is only guaranteed when an initial estimation with respect to the urgency for further treatment can be made immediately on admittance, which structures further treatment. For this purpose nursing-based, structured and validated triage systems are available in hospitals, which are recommended in this position paper of the German Society of Interdisciplinary Emergency and Acute Medicine (DGINA) for application in integrated emergency departments. Comparable triage tools do not exist for the KV emergency practices and the hospital triage has also not been evaluated from the aspect of whether it enables a risk-free referral of patients in another emergency treatment unit with other treatment principles. Based on this starting position, with respect to the triage the DGINA demands that attention be paid to the following 5 points to maintain patient safety in German emergency departments.
