Abstract

Streptococcus pneumoniae is a frequent cause of local and systemic infections in humans. The polysaccharide capsule, of which so far 97 different serotypes have been described, is its most important virulence factor. Elderly people show an increased susceptibility to pneumococcal infections, even in the absence of additional risk factors. In Germany two different pneumococcal vaccines are available for prophylaxis: the 13-valent polysaccharideprotein- conjugate vaccine (PCV13) and the 23-valent polysaccharide vaccine (PPSV23). Today, following the widespread use of PCV13 in infants and small children, only 30 % of invasive pneumococcal disease (IPD) episodes in elderly people (>60 yrs) are caused by pneumococcal serotypes contained in the PCV13 vaccine, whereas roughly 70 % of the IPD cases are due to serotypes covered by the PPSV23 vaccine. As in immunocompetent adults and elderly people both vaccines showed a comparable efficacy against pneumococcal disease with vaccine serotypes, the Standing Committee on Vaccination in Germany (STIKO) recommends the use of PPSV23 as the standard anti-pneumococcal vaccine in people >60 yrs of age. In the presence of certain underlying diseases the sole application of PPSV23 or the sequential vaccination with PCV13 followed by PPSV23 after a minimum of 2 (better 6-12) months is indicated. This review will discuss the principal immunological differences between both vaccines and the considerations that led to the recommendation by the STIKO.
