Abstract

Syncope is a transient loss of consciousness, which suddenly occurs and is explained by a short global hypo perfusion of the brain. Patients presenting with syncope or near syncope to acute care display increased morbidity and mortality. Evaluation of patients with syncope or near syncope is complex due to the heterogeneity of the syndrome and the heterogeneous presentation of affected patients. Subsequently, diagnostic efforts for syncope evaluation of affected patients sometimes are not productive ("shotgun approach"). Moreover, technical examinations such as cranial computer tomography or echocardiography are often not necessary and should only be ordered in the presence of an indication in a focused manner. We therefore suggest implementing a standardized way of diagnostics and risk stratification as suggested by guidelines in patients with syncope or near-syncope presenting to emergency care. This should always include patient history, physical examination, 12-lead ECG, orthostatic testing and laboratory evaluation. Further diagnostics are usually not necessary and should only be ordered, if there is clinical suspicion of certain risks. Assessment tools may support the acute care physician to identify patients at risk for adverse outcome. However, physicians should be aware of the low sensitivity and specificity of respective tools. Therefore, the disposition decision of affected patients should be made on individual evaluation of affected patients by the responsible emergency physician. © 2013 Dustri-Verlag Dr. Karl Feistle.
