Abstract

Epilepsy surgery has become accepted as a successful therapeutic option in patients with drug-resistant focal epilepsy and was comprehensively reviewed in the August 2016 issue of this journal [16]. As a general conclusion, patients with a structural brain lesion as the likely trigger for the patient’s drug-refractory epilepsy are candidates for a successful surgical therapy. Neuropathology is mandatory to identify and classify any structural brain lesion in surgically resected tissue samples in order to assign a scientifically validated diagnosis; however, the spectrum of epilepsy-associated brain lesions is broad, covering neoplasms, neuronal cell death and degeneration (hippocampal sclerosis), malformations of cortical development, glial scars, vascular malformations and inflammation. The neuropathology reference center for epilepsy surgery in Erlangen identified 36 histopathological entities in a series of 5622 patients, whereby the 10 most common disease entities accounted for 84.3% of the entire cohort. More than half of all the patients had either hippocampal sclerosis, ganglioglioma or type II focal cortical dysplasia (total of 54.3%). These figures and numbers also highlight typical patterns for localization of the lesion and seizure onset. Together with previously published series of post-surgical long-term seizure control, the neuropathological investigations and evaluation of large patient collectives provide a reliable basis for prediction of the most suitable candidates for epilepsy surgery.
