Abstract

The treatment of leg ulcers requires a comprehensive assessment of the patient and the underlying pathogenesis and is subject of multimodal concepts. After unsuccessful conservative treatments, the surgical wound debridement is the treatment of choice. This is complemented by local vacuum therapy or in case of chronic infected wounds by applying an instillation therapy. After adequate wound conditioning and need for plastic surgical reconstruction, minimally invasive procedures such as skin transplantation in superficial defects, up to microsurgical tissue transplantation in severe losses of substance are available. If a perfusion disorder of the affected limb is presented, a revascularisation by bypasses or AV-loops is required. With new methods in wound healing, the success rate and therefore also the quality of life of patients with chronic wounds may be improved.
