Background The Information Network of Departments of Dermatology (IVDK), currently with 56 members (dermatological clinics and offices), is dedicated to clinical epidemiology of contact allergy. Analyses of frequencies of contact sensitization in the patch tested patients are one topic of interest. Objective. To present current frequencies of sensitization and recent developments based on IVDK data of the years 2015 - 2017. Material and methods. Retrospective analysis of IVDK data, 2015 - 2017, and partly before. Results. (I) Nickel is still by far the most frequent contact allergen; sensitization frequency among young women, however, is significantly declining. (II) Proportion of positive reactions to fragrance mix is decreasing since 2012. The addition of sorbitan sesquioleate, an emulsifier also contained in fragrance mix, into the DKG baseline series facilitates interpreting positive test reactions to fragrance mix. (III) After the peak of the epidemic of contact allergy to methylisothiazolinone (MI) in the years 2013 and 2014, frequency of sensitization is declining thanks to the limitation of the use of MI in cosmetics. (IV) The increase of the patch test concentration of the preservative methyldibromo glutaronitrile (MDBGN), which is prohibited in cosmetics since 2008, resulted in an increase of positive test reactions, most additional reactions probably being false positive. Conclusion. EU-wide measures to reduce liberation of nickel from costume jewellery and restrict usage of MI in cosmetics have shown some impact. The reason for the decline of positive reactions to fragrance mix remains unclear. Patch testing with MDBGN at 0.3% in petrolatum may yield false positive reactions.
