Standardised measurements in exercise therapy: the status quo within German medical rehabilitation

Abstract

Background: The use of standardised measurements (SM) in exercise therapy has been recommended to encourage high-quality, evidence based practise. However, the use of SM within exercise therapy in Germany has not been investigated.

Objective: The purpose was to analyse the distribution, content and specific purpose of SM during exercise therapy practice in the context of medical rehabilitation.

Method: Observational, cross-sectional study. Germany-wide written survey in medical rehabilitation including 1,558 exercise therapy departments. The survey questionnaire covered exercise therapy concepts and processes containing 10 measurement-related questions. Measurements were classified as standardised or non-standardised. Meaningful item level concepts from SM were identified and allocated to the ICF categories deductively by using the revised ICF-Linking Rules. Two independent raters carried out all ratings and categorizations.

Results: 576 of 707 (81%) exercise therapy departments collect information on rehabilitation patients. 1,008 measurements (54%) were classified as SM. On average departments apply 1.4 SM (SD: 1.8) and 53% use at least one SM. The 1,008 SM included 9,007 concepts on the ICF: function/ structure (49%), activity/ participation (4%), activity and function (26%), person-related (16%) and environmental (1%) factors.

Conclusion: The use of SM within exercise therapy in Germany is relatively widespread but not obligatory. The focus of measurements is to assess physical functions and activities whilst other important determinants of physical activity behaviour are rarely assessed.
